First Appointment

55 minute initial appointment for all new clients – $200

Individual Session

55 minute individual session – $165

Consultation or Supervision

$165 per hourly appointment

Relationship Session

85 minute relationship session for couples or expanded individual session – $200 (please note this session option is not reimbursable by insurance)

Insurance

I am paneled with the following insurance plans:

This means I am an in network provider (INN).  We will verify benefits to determine whether you have a deductible and how much has been met, and whether you have a co-pay or co-insurance to be collected at each session.

For all other insurance panels, I am an out of network provider (OON).  I will provide you with a “superbill” for services, which is a receipt your insurance provider requires for you to be reimbursed according to their plans.  Out of network benefits vary according to each plan type.  Please contact your insurance provider to determine what your particular reimbursement rate will be.

You can find the contact number for your insurance on the back of your insurance card and it may be labeled “customer service” or “member services.”  You will need to have the name of the insurance holder, the insurance holder’s date of birth, the member ID and the group number.  You can find the member ID and the group number on the front of your insurance card.

Payment

Payments are collected at each session in the form of cash or credit/debit card (including HSA/FSA).  Credit card authorizations are stored securely online through an electronic health record system and may be used in the event you do not have the physical card on hand and would like to pay by credit.

Payment is due at the time of each session.

What is my deductible?
(Please note that you may or may not have a deductible depending on your plan.)

Do I have a co-pay or co-insurance due at each visit, and if so, what is it?
(Again, you may or may not have a co-pay or co-insurance for each session.)

Is there a maximum limit of sessions per year?
(There may be session limits imposed by your insurer.)

Do I need a pre-authorization for sessions?
(A pre-authorization is a decision by your insurer that your treatment is medically necessary. Not all insurance companies require pre-authorizations.)
